Damage/Loss/Theft of UNIVERSITY ID CARDS
If you require a replacement University Card, please complete the form below.
Charges (these will appear on your college bill):

| Lost cards/stolen cards | £11, unless you provide a Police Crime Reference number |
| Damaged cards | free, provided you return your previous card to the Tutorial Office (Essex 5), when you collect your new card |
- By submitting a request for a replacement card, you agree to pay the charge, so please do check that you need one, before we order a replacement.
- The Tutorial Office will notify you when your card is ready to collect.
- You will then need to arrange to have it reprogrammed for access by the Queens' Porters, and by your department/faculty for any non-college access.
